Disclaimer forms
after our discussion last night re: contraindications, medical approval etc and latest guidance from IGPP I'd be grateful if anyone could fwd me a copy of a disclaimer form they use regularly with clients. possibly this is combined with your notes/client record, but thought worth seeing how others do it before creating my own. Does anyone know if embody have such a thing on web too?
