Consultations at fairs/fund raising events?

Hello Please could you help? I'm offering hand massages at my local church to raise funds. I know I need to go through some questions before I start the massages. Would you recommend I just adapt my standard consultation form? I've got limited time - 15 mins each person for the treatment. Would it be OK for me to just ask for their contact info, if they have any medical conditions, if they are on any medication, and to explain the healing crisis? Thats pretty much what I have done but you might want to check the insurance situation as there is a greater sense of caution around now RG

I normally also check for specifics like blood pressure problems and diabetes (in case a massage alters blood sugar levels) - but I think for 15 mins you can't really do very much!. Better to spend more time on the massage & hope you can recruit them to a full treatment! Amanda - Milford
