contra indications

Does anyone have a template of a contra indications form for clients to sign at pamper parties etc ? Some of the mini treatments I offer would be too long if I went through everything as I would a full hour appointment (but of course clients (and I) need to be aware). Thanks

Hi When I have done pamper evenings etc, I have asked the client if there are any health concerns that I need to know about and give examples.

i usually have a poster and flyer with a list of things i need to know and if its a party i get the clients to just sign a form saying they have no contras if you send me an email direct i wil lmail you my tastersession form barb@b-l-t.co.uk and poster barb
