contra indications

Does anyone have a template of a contra indications form for clients to sign at pamper parties etc ? Some of the mini treatments I offer would be too long if I went through everything as I would a full hour appointment (but of course clients (and I) need to be aware). Thanks

Hi When I have done pamper evenings etc, I have asked the client if there are any health concerns that I need to know about and give examples.

Dear Victoria, Please find to follow a link for a form which we use when we do the North Kent and South East London CThA pamper evening (we call it Try A Therapy evening). Very quick and inclusive. http://www.sportmassagetherapy.net/Contraindication-P16.html I hope it will help. Best regards, Giuseppe
