Electronic/Paper Free Consultation Form
Hi everyone. Hope you're all well. Curious to know if anyone is using an online/electronic and paper-free consultation form they would recommend? Something a client can fill in and sign electronically when booking a treatment to reduce the need for paper/contact? Anything spring to mind?

Hi, Jotforms, is easy to use and you can do electronic signatures, you can create up to 5 different types of forms within the free version. Also, there are Cognito and Goggle forms but I think you have to pay if you want the electronic client signature.

Did you find a solution for this? I am currently in the process of setting up and would be interested to know what you found. I have created a consultation form in google forms but cannot find an "user friendly" workaround for getting the signature. I wonder if a declaration that they opt to agree to would be enough!?

Apologies for the delay in response. I went ahead with Jotforms. The only issue I e come across is where a client is not technically savvy. You'd still need a paper back up, or help them complete the form in person.
