About massage insurance

Hello, I'm Miku ZURNIA. I have some questions. Can I get the massage insurance? What the insurance policy covering? Could you let me know how much it would be? My maiden name was IKUTA. When I got these certificate I was single, after got married my family name changed to ZURNIA so I will send my marriage certificate. Miku ZURNIA
