Consultation Forms

I have a new client who is objecting to completing a consultation form. I have never come across this before and was hoping for some guidance with regards to my reply.

Hi Denise,
Thanks for your message.
We would advise you not to treat any customer without getting them to fill the Consultation form.
Because, it will be a breach in CThA code of practice and your membership will be jeopardised.
Your Insurance will not be valid either, you will not be covered.
